Fill Out Your Pre-Visit Questionnaire

It’s helpful for your lactation consultant to know how many wet and dirty diapers your baby has had in the past 24 hours, the number of times they have breastfed, what their official weights are from the pediatrician’s office and if baby has received any supplemental expressed breastmilk or formula. Having this information ahead of your appointment will enhance your visit.

We ask that you submit your information by filling out your Pre-Visit Questionnaire. Pre-Visit

Gadgets

If you’re using any breastfeeding gadgets, such as a nipple shield or Supplemental Nursing System, please bring them to the appointment.

Missed Appointments

Life happens, so we understand that sometimes you can’t make your appointment.

Please call us at least one full business day in advance to cancel or change an appointment.

If you don’t call to cancel in advance, we reserve the right to charge a No-Show Fee (currently $20) to cover the cost of the unfilled appointment slot.
